Your annual exam fee includes extended visits, the annual comprehensive exam (EKG, PFT, vision, hearing, cardiovascular risk assessment), chronic disease management, personalized care planning, direct physician communication, same or next-day appointments, and care coordination with specialists and imaging.
No hidden fees. All costs are listed upfront. Any additional charges are discussed in advance.
You can update or cancel your plan anytime. Contact us for help with changes.
Yes — we will be credentialed with most major insurance plans, including Medicare, and bill insurance for covered medical services in our office. Your annual exam fee is separate and is not billed to insurance. Depending on your plan, some services may also be HSA/FSA eligible; we can provide documentation on request and recommend confirming details with your plan administrator.
